약물 중독이나 화학 물질 중독 응급 치료용 약물, Medicines for treatments for drug overdose or chemical poisoning

가정에 보관했다가 필요에 따라 응급용으로 쓸 수 있는 약물은 이페칵 시럽, 활성 탄말이다.

1. 이페칵 시럽 Ipecac syrup

의사의 지시에 따라서 이 약물을 보관하고 그의 지시에 따라서 필요할 때 써야 한다.

  • 약물이나 독극물 또는 화학 물질 등 잘못해서 경구로 섭취했거나 자살 기도를 하기 위해 먹고 중독될 수 있다. 
  • 화학물질이나 약물을 경구로 먹은 후 그런 물질에 중독될 수 있다고 의심되거나 중독되었다고 판단하면 종합병원 독극물 중독치료센터 또 병원 응급실, 독극물 중독 치료센터(미국의 경우 1-800-222-1222), 단골 의사에게 응급으로 전화상담 치료를해 그들의 지시에 따라 치료를 현장에서 시작하고 단골 의사, 독극물 중독치료센터, 또는 응급실의 지시에 따라 즉시 적절한 치료를 현장에서 시작해야 한다. 
  • 필요에 따라 구토를 시킨다든지 의료 구급대 등의 도움을 받아 가장 빠르고 편리한 교통수단을 이용해서 응급실애서 그 다음 추적 응급 처치를 받아야 한다.
  • 2003년 전까지, 이페칵 시럽을 가정에 보관했다가 독극물을 경구로 섭취해서 독극물에 중독이 생길 가능성이 있으면 경구 이페칵 시럽 투여로 위 속 약물이나 독극물을 구토해 독극물 응급 치료를 해도 된다고 미 소아과 학회는 권장했었다. 그러나 미 소아과 학회는 2003년부터 이페칵 시럽을 가정에 통상적으로 보관하지 말라고 권장한다. 
  • 그 이유는 다음과 같다. 

① 소아 독극물 중독을 이페칵 시럽으로 가정에서 응급으로 치료해도  이페칵 시럽 치료의 전반적 결과는 이페칵 시럽으로 치료받지 않았을 때보다 더 좋은 결과가 생기지 않았고, 

② 이페칵 시럽 치료를 해도 섭취한 독극물 내용물 등을 다 구토해 내지 않았고,

③ 중독이 되지 않는 물질을 먹었는데도 부모가 무분별적으로 이페칵 시럽 응급치료를 하고, 

④ 의사와 상의도 하지 않은 상태에서 부모의 마음대로 이페칵 시럽으로 독극물 중독을 치료하고,

⑤ 부모가 가정에서 이페칵 시럽으로 독극물치료를 이미 했던 경우, 때로는 구토를 계속해서 이페칵 치료로 필요한 경구 투여 치료약을 경구로 투여 치료할 수 없다는 것이다. 

이런 이유로 이페칵 시럽을 집에 통상적으로 보관했다가 독극물을 경구로 섭취해서 독극물 중독되는 것을 가정에서 치료하라고 더 이상 권장하지 않는다. 

그러나 사는 나라의 의료 상황에 따라 

 가정에서 보관 할 수 있나 단골 소아청소년과에 알아보는 것이 좋다.

  • 때로는 병원 응급실에서 필요에 따라 이페칵 시럽을 경구로 투입하든지 코 인두 식도 카테터(관)를 위 속에 넣고 그 관을 통해 이페칵 시럽을 위 속에 주입해서 위내 중독 약물이나 독극물 등을 10~15분 내에 토하게 해서 독극물 등으로 중독되지 않게 예방 및 치료하기도 한다. 
  • 이페칵 시럽의 1회 치료용량은 나이에 따라 다르다. 제약 회사의 투약용법에 따라 투약해야 한다. 
  • 적절한 용량을 경구로 투여한 후 10~15분 정도 되면 구토하는 것이 보통이다. 
  • 이페칵제는 이페칵 시럽 형태와 이페칵 농축액 형태의 두 가지가 있다. 
  • 처방에 따라 그 중 이페칵 시럽을 집안에 보관하고 있다가 의사와 상의한 후 필요에 따라 쓸 수 있다. 
  • 농축액 형태의 이페칵 액을 쓰면 이페칵 농축액 중독에 걸린다. 
  • 집안에 이페칵 농축액을 의사의 지시 없이 보관해서도 안 되고 그 농축액 형태의 이페칵으로 약물 중독 등을 치료해서도 안 된다.

  • 약물이나 독극물 등을 우발적으로, 또는 다른 이유로 먹은 후 중독될 것 같다고 의심되면 의사의 지시에 따라 집안에 보관했던 이페칵 시럽 15~30cc 정도를 경구로 먹여 치료할 수 있다. 
  • 그 외 다른 이유로 위 내 내용물이나 먹은 것을 급히 토해내어야 할 때도 이페칵 시럽을 의사의 지시에 따라 사용할 수 있다.
  • 휘발유, 석유, 또는 등유 등의 화학물질을 잘못해 먹었을 때는 일반적으로 이페칵 시럽으로 구토를 시켜 치료해서는 안 된다. 
  • 석유나 광유를 소량 먹었을 때는 이페칵 시럽으로 토하게 하여 치료할 수 있다. 이 역시 의사의 지시와 판단에 따라 치료해야 한다. 
  • 양잿물 또는 황산 등 강한 산이나 강한 알칼리 물질을 잘못해 마시거나 먹었을 때는 이페칵 시럽으로 구토시켜 치료해서는 안 된다. 
  • 어떤 병으로 구토를 하거나 구토 반사를 잃었거나 환자가 경련을 하거나 혼수상태에 빠져 있을 때는 이페칵 시럽으로 구토 시켜 치료해서는 안 된다. 
  • 가능한 한 이페칵 시럽을 경구로 먹여 독극물 중독을 치료하기 전에 병원 응급실, 단골 의사, 또는 독극물 치료 센터의 지시에 따라 이페칵 시럽으로 치료해도 좋은지 알아보고 그들의 지시에 따라 치료 시작하는 것이 이상적인이다. 
  • 독극물이나 과량의 약물을 잘못해서 경구로 섭취했을 때 섭취한 독극물을 토하게 하여 치료하는 방법 이외 더 많은 다른 치료를 해야 할 때가 많고 필요에 따라 다음 단계의 독극물 치료가 필요하다. 
  • 의사의 지시에 따라 이페칵 시럽으로 구토를 시킨 이후 종합병원 응급실, 단골 의사, 독극물중독치료센터 등의 지시에 따라 구급차로나 다른 적절한 교통수단을 이용해서 병원 응급실이나 독극물치료센터로 급히 데리고 가야 한다.

  1). 이페칵 시럽과 독극물 중독사고 치료

  • 1940년도에는 미 소아들이 독극물중독사고로 500여명이 사망했고 1997년에는 25명이 사망했다. 
  • 소아가 안전사고로 약물이나 독극물을 섭취해 중독이 될 것 같든지 이미 중독이 되었으면 의사의 지시나 응급실 또는 독극물 응급 치료센터의 지시에 따라 그 아이에게 이페칵 시럽을 먹여 독극물을 토해 치료했었다.
  • 그래서 소아들이 있는 모든 가정마다 30cc 이페칵 시럽 한 병을 보관하라고 권장했었다. 그래서 최근에는 다음과 같이 그 치료 방법을 달리 권장하고 있다.

①미 소아과 학회에서는 영유아들과 학령기 아이들의 건강관리를 하는 의료인들은 소아들에게 독극물 섭취로 인한 독극물 중독이 생기지 않도록 부모들에게 통상 교육을 시키라고 권장하고 

②소아에게 생긴 독극물 중독을 이페칵 시럽으로 전에 권장했던 것과 같이 통상적으로 치료해서는 안 된다고 권장하고 

③의사들이나 소아들의 건강관리를 하는 의료인들의 지시에 따라서만 이페칵 시럽으로 응급치료를 해야 한다고 권장하고 

④부모들이 의사의 지시 없이 독극물중독 치료용 활성 탄 말도 통상적으로 가정에 보관하는 것도 권장하지 않는다. 

⑤독극물 중독이 생길까 의심될 때는 독극물 중독 치료센터 1-800-222-1222에 전화해서 그들의 조언과 치료를 받아야 한다고 독극물 응급치료 정책을 최근에 발표했다.

또 이페칵 시럽으로 토하도록 하여 이페칵 시럽을 “대리인에 의한 먼차우센 증후군”을 일으킨 예도 있다. 

  • 독극물 치료를 잘못한 경우도 있고 이페칵 시럽으로 구토해서 비만 체중을 치료하는 예가 있기 때문에 가정에서 더 이상 이페칵 시럽을 보관하는 것도 권장하지 않는다고 발표했다.

2. 활성 탄말 Activated charcoal powder

  • 활성 탄 말은 일종의 숯가루이다. 
  • 안전사고로 또는 다른 이유로 과량 약물 섭취, 또는 독극물을 섭취했을 때 활성 탄 말을 먹으면 활성 탄 말에 위장관 내 세균, 약물 또는 독극물 등이 흡수될 수 있다. 
  • 활성 탄 말이 위장관 속에 있는 약물, 독극물 등에 접촉될 때 그 약물이나 독극물 또는 위장관 내 세균 등이 활성 탄 말로 흡수될 수 있다. 
  • 활성 탄 말제는 분말 형태와 현탁액 형태가 있다. 
  • 미소아과 학회에서는 분말 활성 탄 말제나 현탁액 활성 탄 말제를 가정에 보관하는 것을 원칙적으로 권장하지는 않는다. 
  • 사는 곳의 여러 가지 조건과 상황에 따라 의사와 상의한 후 집안에 보관했다가 잘못해서 먹은 독극물, 화학
  • 물질 또는 약물 등으로 중독될 수 있다고 판단하면, 또는 의사의 지시에 따라 집에서 응급처치를 할 때 쓸 수 있다. 
  • 위장관 내 독극물이나 약물 등이 핏속으로 흡수되기 전에 활성 탄 말을 경구로 먹거나 비위 세관 등을 통해 위장관 속에 주입하면 독극물 등이 위장관 속에 넣은 활성 탄 말로 흡수되어 독극물 중독이 생기지 않게 예방적 치료를 할 수 있고, 또 이미 생긴 중독의 병세를 경감시킬 수 있다. 
  • 환아 자신이 활성 탄 말을 경구로 섭취할 수 없을 때는 비 위 세관을 콧구멍→비강→인두→식도를 통해→위 속에 넣고 그 관을 통해 활성 탄 말을 위장관 속에 넣어 독극물 중독이나 약물 중독을 예방, 치료할 때도 있다. 
  • 세관을 입을 통해 인두 식도 위 속에 넣을 수도 있다.
  • 활성 탄 말 현탁액은 15gm이나 25gm 또는 50gm를 병에 담아 판다. 
  • 이 세 가지 크기의 활성 탄 말 현탁액 병을 하나를 사서 가정에 보관하고 있다가 필요에 따라 의사의 지시에 따라 쓰면 좋을 것이다. 
  • 의료시설이 미비한 산촌 벽지나 시골에 사는 소아에게 중독사고가 나서 치료 차 병원에 도착하는 데 많은 시간이 요할 때를 생각하면 이페칵 시럽과 활성 탄 말을 가정에 보관했다 독극물사고가 나든지 난 것을 의심하면 소아청소년과나 병원 응급실의 지시에 따라 이런 약으로 집에서 응급 치료를 할 수 있다. 
  • 첼코에이드쥐나 액티도우즈-아쿠아 등은 일종의 활성 탄 말 현탁액의 상품명이다.

3. 그 외 중독 치료 약물 The other drugs for poisoning treatment

  • 위장관을 자극할 수 있는 화학 물질이나 부식성 화학물질을 먹었을 때 맹물이나 우유 등을 먹이면 위장관 속 물질이 희석되게 응급치료를 할 수 있다. 
  • 그러나 이런 치료 역시 의사의 지시에 따라 응급으로 치료하는 것이 원칙이다. 
  • 지연성 흡수를 요하는 약물로 중독이 생길 가능이 있을 때는 황산나트륨(Sodium sulfate)등 설사 하게 하는 약물로 치료할 수 있다. 
  • 또한 이런 치료 역시 의사의 지시에 따라 하는 것이 원칙이다.

④ Treat poison poisoning with Ipecac syrup at will, without consulting a doctor.

⑤ If the parents have already taken poison treatment with Ipecac syrup at home, sometimes they continue to vomit, and it is impossible to orally administer the necessary oral treatment for Ipecac treatment. For this reason, it is no longer recommended to treat poison poisoning at home due to oral ingestion of poison after conventional storage of ipecac syrup at home. However, depending on the medical condition of the country where you live It is good to check with your regular pediatrician to see if it can be stored at home.

• Sometimes in the hospital emergency room, if necessary, either injecting Ipecac syrup orally or inserting a nasal pharyngeal esophageal catheter (tube) into the stomach and injecting Ipecac syrup into the stomach through the tube to instill intragastric poisoning drugs or poisons for 10 to 15 minutes Prevents and treats poisoning with poisons by causing vomiting inside.

• If the patient is vomiting or has lost the vomiting reflex for any illness, or if the patient is convulsing or in a coma, do not treat vomiting with Ipecac syrup.

• If possible, prior to treating poison poisoning by administering ipecac syrup orally, it is ideal to follow the instructions of the hospital emergency room, regular physician, or poison center to determine if it is ok to treat with ipecac syrup and start treatment as directed.

• When a poison or an overdose of a drug is ingested orally by mistake, more treatment is often required other than the treatment by causing the ingested poison to vomit, and if necessary, the next level of poison treatment is required.

• After vomiting with Ipecac syrup according to the doctor’s instructions, follow the instructions of the general hospital emergency room, regular doctor, poison poisoning treatment center, etc. .

1). Treatment of Ipecac Syrup and Poisoning Accidents
